Medical Records                                 

 Your Health Information

If you have received services at the Health Department, you have a medical record that contains your health information.  In order access this information, a signed authorization is required    from the patient or legal guardian.  Information may also be released with a valid court order or subpoena commanding release of medical information.  The patient is legally entitled to review their own records and may obtain copies for a fee.  Records are retained for ten years after the last date of service.

Fees for Copies of Medical Records

$2.00 for the first page and $ 0.75 for each additional page up to a total of 25 pages ($20.00).  There is no charge for copies 26 – 40, and any additional pages over 40 are charged at 
$ 0.25 per page.  If information is released to another health care provider, there is no charge.
