- Immigration medical forms may be picked up approximately four
weeks after initial visit. (see FAQ below)
Please bring the following information:
Picture Identification All medical forms will be filled
out with the name that appears on pictured I.D. unless a
female that has recently married brings her marriage certificate
and requests that the medical form is filled out in her married
Immunization Records All records of MMR (Measles, Mumps
and Rubella), TD (Tetanus and Diphtheria) Varicella (Chicken
Pox) 1st and 2nd dose. If 65 years of age or older, you will
need proof of influenza and pneumonia vaccine or you will
get vaccinated at your visit.

If medical records are not in English please have them translated
before presenting for medical visit.
