Renal Disease Intervention Project
The function of the Renal Disease Intervention Project is to identify and to provide home visits to persons at risk for developing End Stage Renal Disease (ESRD) due to diabetes and/or hypertension. A registered nurse provides counseling, referral and educational services to these persons in order to improve their understanding of the disease process, diet, medications and appropriate lifestyle modification. A degreed social worker and registered dietician are also available as needed to consult with the patients.
