Welcome to the web site for Case Management Services (CMS). CMS is staffed by a very diverse group of nurses, social workers, social counselors, case managers, health educators, lay-outreach workers, and support staff.
The programs within the department are as diverse as the staff. They serve citizens of Hamilton County ranging in ages from newborns to the very oldest. The department is unique in that all of the programs are home visiting programs. CMS bilingual staff are available to assist with our Hispanic/Latino population.
Two new state-funded programs have been added to the CMS Department. These are the Lead Poisoning Prevention and the TENNderCare Outreach Programs.
The department is also involved in the Health Department’s emergency preparedness planning.
