Welcome to Case Management Services (CMS)
CMS is a home visiting program staffed by a diverse group of nurses, social workers, social counselors, case managers, health educators, lay-outreach workers, and support staff. CMS serves the citizens of Hamilton County, from newborns to the very oldest. CMS bilingual staff are available to assist the Hispanic/Latino population.

Childhood Lead Prevention
Lead poisoning is considered to be the most serious environmental threat to children's health. The Lead Prevention Program provides education to the community about this threat and does case management when children are screened and found to have an elevated blood lead level.

Newborn Screenings for Genetics allows for early diagnosis of metabolic disorders and hearing loss, followed by appropriate medical treatment. All babies born in Tennessee are screened for certain genetic conditions. We provide education for the parent(s) about the screening and what options they have if the newborn needs further testing and/or medical assistance.

Help Us Grow Successfully (HUGS) is a home visitation program which through intervention, assessment, and education, assists pregnant and postpartum women for up to two years, and infants and children up to age six in gaining access to medical, social, and educational services.

Children's Special Services provides medical case coordination and services to eligible children with special health care needs. CSS has three components: medical services, care coordination, and support identification.

Sudden Infant Death Syndrome (SIDS) The SIDS Program provides education and case management for families who have lost an infant due to SIDS.

TENNderCare encourages families to get their Well Child Check Ups and health care services for children ages birth to 21 who have TennCare.
PAFT -  Parents Are First Teachers
A program to teach parenting skills and early childhood education information to parents in Hamilton County who have children, prenatal to age 5.

HIV/AIDS Medical Care Management Our HIV Medical Care Managers work specifically to get people with HIV/AIDS into the medical system and refer them for other needed services.

Renal Disease Intervention
The function of the Renal Disease Intervention Project is to identify and to provide home visits to persons at risk for developing End Stage Renal Disease due to diabetes and/or hypertension.
