Case Management Services (CMS), a division of the Chattanooga-Hamilton County Health Department, is staffed by a very diverse group of nurses, social workers, social counselors, case managers, health educators, lay-outreach workers, and support staff.

The programs within the department are as diverse as the staff. They serve
citizens of Hamilton County ranging in ages from newborns to the very oldest. The department is unique in that all of the programs are home visiting programs. CMS bilingual staff members are available to assist with our Hispanic/Latino population. CMS is also very involved in the Health Department’s emergency preparedness planning.
