Dental
care is available to the residents of Hamilton County by
appointment. Our primary focus is providing preventive, restorative, and
emergency care to patients
4-20 years of age. Adults
are also seen for emergency dental care.
Services are provided on a sliding fee scale based on family income.
TennCare recipients (ages 4-20) may also be seen for basic
services with no additional charge.

After
an appointment is scheduled, you may download the patient registration
form, the patient health history form, and the medical
information/consent release form. Complete
the forms and bring them to the appointment.
A parent or guardian must sign the forms if patient is under
18 years of age. If a
guardian signs the forms, custody papers must be brought to first
appointment.
