Dental care is available to the residents of Hamilton County by appointment.  Our primary focus is providing preventive, restorative, and emergency care to patients 4-20 years of age.  Adults are also seen for emergency dental care.  Services are provided on a sliding fee scale based on family income.  TennCare recipients (ages 4-20) may also be seen for basic services with no additional charge.

8:00 a.m. – 4:00 p.m.        Monday – Friday

After an appointment is scheduled, you may download the patient registration form, the patient health history form, and the medical information/consent release form.  Complete the forms and bring them to the appointment.  A parent or guardian must sign the forms if patient is under 18 years of age.  If a guardian signs the forms, custody papers must be brought to first appointment.
