Primary Care Services
Provides sick care for those with illness-related symptoms who may need exams, tests, and treatment;
as well as those seeking wellness and preventative care.

The following services are provided for family members of all ages.

  • Sick care for acute symptoms and illness
  • Complete physical exams
  • Immunizations
  • Women’s health/contraceptive care
  • WIC services
  • Health education and counseling
  • Preventative screening and tests

Fees are charged for all primary care services; services may be discounted based on your family size and income (sliding scale fee).

Family Health Clinic
921 E. Third Street, Chattanooga
Appointments: (423) 209-8050
Hours: Monday-Thursday 8 a.m.-5 p.m. and Friday 9 a.m.-4 p.m.
Fees/Payments: TennCare accepted; sliding scale fee services offered.

  • WIC
  • Childhood immunizations
  • Newborn genetic screenings (PKU, Sickle Cell Anemia, Thalassimia, Hypothyroidism)
  • Examination for Pediculosis (head lice)
  • Immigration physicals for children

Adult/prenatal area:

  • Pregnancy testing
  • Family planning
  • Emergency contraception
  • Prenatal care-Basic & Full levels
  • Routine adult immunizations (MMR, TD)
  • Breast and cervical cancer screening program

Birchwood Clinic

Hours: Monday and Friday 8 a.m.-4 p.m., Tuesday and Thursday 8 a.m.-3 p.m., and Wednesday 8 a.m.-6 p.m.
