Primary Care

Primary Care Services provide sick care for those with illness-related symptoms who may need exams, tests, and treatment, as well as for those seeking wellness and  preventive care. 

Services provided include the following for family members of all ages:

  • Sick Care for Acute Symptoms and Illnesses

  • Complete Physical Exams

  • Immunizations

  • Women’s Health/Contraceptive Care

  • WIC Services

  • Health Education and Counseling

  • Preventive Screenings and Tests

Fees:   Fees are charged for all primary care services, however services may be discounted based upon your family size and income (sliding scale fee).

Services are available at the following locations: 

 

Birchwood Health Center 

               Location:          5623 Hwy. 60, Birchwood, TN

            Appointments:              (423) 961-0446

            Hours:  Monday           8:00 – 4:00 pm 

                        Wednesday      8:00 – 4:00 pm

                        Friday              8:00 – 4:00 pm

             
Fees/payments: TennCare accepted; Sliding scale fee services offered. 

             Services Offered: WIC, childhood and adult immunizations, newborn genetic screenings (PKU, Sickle Cell Anemia, Thalassemia, Hypothyroidism), examination for pediculosis (head lice), pregnancy testing, family planning, emergency contraception, preventive health care and sick care for families, health education and counseling, preventive screenings and tests, and the Breast and Cervical Cancer Screening Program. 

Homeless Health Care Center 

            Location:          717 E. 11th Street, Chattanooga, TN

            Hours:  Monday           7:30 – 4:30 pm

                        Tuesday           7:30 – 4:30 pm

                        Wednesday      7:30 – 4:30 pm

                        Thursday          7:30 – 4:30 pm

                        Friday              7:30 – 4:30 pm 

            Fees/Payments: TennCare accepted; no charge for qualifying individuals.

Ooltewah Clinic
