Welcome to Case Management Services (CMS). CMS is staffed by a very diverse group of nurses, social workers, social counselors, case managers, health educators, lay-outreach workers, and support staff.

The programs within the department are as diverse as the staff. They serve citizens of Hamilton County ranging in ages from newborns to the very oldest. The department is unique in that all of the programs are home visiting programs. CMS bilingual staff are available to assist with our Hispanic/Latino population.
  


Case Management Services and Programs
 




Adult Health
The Adult Health Program provides oversight management to persons over 18 years of age who have no available caregiver and do not qualify for home health care reimbursable by health insurance.

Childhood Lead Prevention

Leading poisoning is considered to be the most serious environmental threat to children's health.  The Lead Prevention Program provides education to the community about this threat and does case management when children are screened and found to have an elevated blood lead level.

 


Children Special Services (CSS)
Children's Special Services provides medical case coordination and services to eligible children with special health care needs.  CSS has three components: medical services, care coordination, and support identification.
 
Help Us Grow Successfully (HUGS)
HUGS program is a home visitation program which through intervention, assessment, and education, assist pregnant women, postpartum women for up to two years, infants and children up to age six in gaining access to medical, social, and educational services.

Newborn Screenings for Genetics

Newborn screening allows for early diagnosis of metabolic disorders and hearing loss, followed by appropriate medical treatment.  All babies born in Tennessee are screened for certain genetic conditions.  This program provides education for the parent(s) regarding this screening and what options they may have if the newborn is found to need further testing and/or medical assistance.

Renal Disease Intervention
The function of the Renal Disease Intervention Project is to identify and to provide home visits to persons at risk for developing End Stage Renal Disease due to diabetes and/or hypertension.

 


Sudden Infant Death Syndrome (SIDS)
The SIDS Program provides education and case management for families who have lost an infant due to SIDS.


TENNderCARE

This program encourages families to get their Well Child Check Ups and health care services for children ages birth to 21 who have TennCare.

HIV/AIDS Medical Care Management
Our HIV Medical Care Managers work specifically to get people with HIV/AIDS into the medical systems and refers them for other needed services.
