COMMUNITY ASSESSMENT AND PLANNING 

The Community Assessment and Planning Section of the Health Department conducts ongoing community health data and information collection and analyses activities in an effort to understand and monitor the health status and health needs of residents of Hamilton County.  In addition to the identification of health needs of residents, staff with this section also are responsible for identifying resources that can be used to address those needs. 

This program area collects two types of data that focuses upon Hamilton County and its residents: primary data and secondary data. Primary data is that information collected directly from area residents by way of community surveys, interviews and town meetings.  The most commonly used surveys are the Youth Behavior Risk Survey and the Adult Behavior Risk Survey.  The secondary data that is collected is information, data and statistics collected by the Tennessee Department of Health, the Centers for Disease Control and Prevention, the Census Bureau, or other governmental institutions.  Most of the data and information collected is aggregate for all of Hamilton County. 

The data collected and analyzed includes demographic information, socio-economic information, mortality and morbidity information, health provider information, and information about specific health problems.  

This service greatly assists City and County leaders, medical and social service agencies and institutions, the local school system, the business community, neighborhood associations, churches, students from area colleges and universities, and other groups and agencies, in their planning and program development activities as well as with their special projects.  This section is also greatly instrumental in helping the Health Department along with the Regional Health Council by providing an information base for the many decisions made in health planning activities, in decisions made regarding the funding for various health programs, and in decisions made regarding the development of new health programs and services.
