IMMIGRATION PHYSICALS

All physicals are done by appointment only. Immigration medical forms may be picked up approximately four weeks after initial visit. (see FAQ)


PLEASE BRING THE FOLLOWING INFORMATION:

Picture Identification

·         All medical forms will be filled out with the name that appears on pictured I.D. unless 

·        A female that has recently married brings her marriage certificate and request that the medical form is filled out in her married name.

Immunization Records

·    All records of MMR (Measles, Mumps and Rubella), TD (Tetanus and Diphtheria) Varicella (Chicken Pox) 1st and 2nd dose.

·         If 65 years or age or older, will need proof of influenza and pneumonia vaccine or will

·         Get vaccine at visit.  (THIS IS ONLY DURING FLU SEASON OCT. 1 – FEB 28)

Translator if Needed

·        If medical records are not in English please have them translated before presenting for Medical visit.

 

FREQUENTLY ASKED QUESTIONS

·        I have been in the United States for six months, how soon should I make an appointment for a physical? 
o
  You should make an appointment as soon as possible.  Appointment slots fill-up quickly, and it takes four weeks after the physical to have medical forms filled out by the Civil Surgeon.
 

·        Can any medical doctor perform the physical?
o   No, the Immigration and Naturalization Service (INS) designates the doctors that perform this type of physical, there are 3 doctors in the Health Dept that serve the Chattanooga area.
 

·        Why does it take so long for the completion of forms?
o
  The reporting of certain test results plus immunizations that require a second dose generally takes four weeks.
 

·        I work long hours and it will be hard for me to pick up my forms, can someone else pick them up for me? 
o  We encourage you to pick up your forms so that we can go over the test results with you and answer any questions that you might have concerning the exam, but we do allow someone that you designate on a signed release form to pick up these documents for you. 

·        What is the cost of an immigration physical?
o
  Depending on your medical history, it could cost
$220.00 to $350.00
