IMMIGRATION
PHYSICALS
Call 423-209-8250 for additional information or to make an appointment All physicals are
done by appointment only. Immigration medical forms may be picked up
approximately four weeks after initial visit. (see FAQ)
Picture
Identification ·
All medical forms will be
filled out with the name that appears on pictured I.D. unless
·
A female that has recently
married brings her marriage certificate and request that the medical
form is filled out in her married name. Immunization
Records ·
All records of MMR (Measles, Mumps and Rubella), TD (Tetanus and
Diphtheria) Varicella (Chicken Pox) 1st and 2nd
dose. ·
If 65 years or age or
older, will need proof of influenza and pneumonia vaccine or will ·
Get vaccine at visit.
(THIS IS ONLY DURING FLU SEASON OCT. 1 – FEB 28) Translator if
Needed ·
If medical records are not
in English please have them translated before presenting for Medical
visit.
