Adult Health

A program for adults who need a little nursing care, but do not qualify for other programs



 

The Adult Health Program provides oversight management to persons over 18 years of age who have no available caregiver and do not qualify for home health care reimbursable by health insurance.  The program provides services by a registered nurse and a social worker.  Referrals are taken from all sources.  Acceptance into the program is based on current caseload capacity. 

Other eligibility requirements:

·        Must be a resident of Hamilton County

·        Must be under the care of a physician

·        Must not currently be receiving home care

·        Must not be a resident of an assistant living or nursing home facility
