The Healthy Living & Wellness Program is working to prevent chronic diseases and health issues among Hamilton County residents by promoting and creating opportunities for wellness improvement that is tailored to the community’s needs.

Chronic Disease Self-Management Program (CDSMP)

§  CDSMP is an evidence-based self-management program developed by Stanford University researchers to aid participants with a variety of health conditions in maintaining their health and improving their quality of life.

§  Any adult with chronic health conditions such as arthritis, diabetes, heart disease, lung disease, and other ongoing health problems are welcome to participate.

§  What are the benefits? There is strong evidence that participants can improve physical and mental outcomes and quality of life in areas including:

o   Decrease pain and health distress

o   Increased energy and less fatigue

o   Increase physical activity

o   Decreased depression

o   Better communication with physicians

o   Decrease social role limitations

o   Increased confidence in managing chronic disease
