Our HIV Medical Case Managers work to coordinate client’s various medical needs. If you or a loved one is HIV positive and lives in Hamilton County or one of the 13 surrounding counties, you could be eligible for Ryan White Medical services from our Medical Case Management program. You could be eligible for assistance whether you currently have health insurance or not.

Medical Case Managers work to:

• Determine Ryan White Coverage eligibility according to policy guidelines
• Assistance in obtaining Infectious Disease and Primary Care referrals     
• Assistance with paying for insurance premiums, co-payments and deductibles (if eligible)
• Assistance in applying for Social Security Disability
• Assistance in applying for Medication Patient Assistance Programs
• Assistance in obtaining dental care
• Assistance with maintaining linkage to care & medication adherence

• Food Bank
• Psychosocial Services
• Nutrition
