Chronic Disease Self-Management Program (CDSMP)
§ CDSMP is an evidence-based self-management program developed by Stanford University researchers to aid participants with a variety of health conditions in maintaining their health and improving their quality of life.
§ Any adult with chronic health conditions such as arthritis, diabetes, heart disease, lung disease, and other ongoing health problems are welcome to participate.
§ What are the benefits? There is strong evidence that participants can improve physical and mental outcomes and quality of life in areas including:
o Decrease pain and health distress
o Increased energy and less fatigue
o Increase physical activity
o Decreased depression
o Better communication with physicians
o Decrease social role limitations
o Increased confidence in managing chronic disease
