HIV Case Management

If you or a loved one is HIV positive and lives in Tennessee, you could be eligible for Ryan White services by enrolling in our HIV Medical Case Management program. Our Medical Case Managers work to coordinate client’s various wellness and medical needs with the help of Ryan White Part B funding. If you need help with registering for benefits that can help get you into medical care, pay for medical care and medication expenses, and other HIV-related health wellness programs, please contact us.

WHAT WE DO

Medical Case Managers work to:

Determine Ryan White eligibility
according to State policy guidelines.
Assist in obtaining Infectious Disease
and Primary Care referrals.
Assist with linkage to
medical care and medication adherence.
Assist, if eligible, with paying for
healthcare insurance premiums,
co-payments and deductibles.
Assist in obtaining dental care.
Assist in obtaining vision care.
Assist with applying for
Medication Assistance Programs.
