The Healthy Living and Wellness program works to improve health outcomes of Hamilton County residents by:

 

  • Collaborating with community partners to develop and strengthen health policies.
  • Promoting healthy behaviors and well-being across the lifespan.
  • Increasing health literacy and skills to improve health outcomes.
  • Supporting environments that maximize potential for health and well-being.

 





Chronic Disease Self-Management Program (CDSMP) is an evidence-based self-management program developed by Stanford University researchers to aid participants with a variety of health conditions in maintaining their health and improving their quality of life. Any adult with chronic health conditions such as arthritis, diabetes, heart disease, lung disease, and other ongoing health problems is welcome to participate.
